Hi there,
I’m planning on getting Breast Reduction surgery in VIC Australia.
I’m 28, the last time I was measured my bra size was 24E, I have struggled with my weight for years, and have found it very hard to lose weight whilst my breasts are this large. 
I cannot run, I cannot sleep on my back, my back hurts, my neck hurts, I have migraines and I struggle with severe Depression. I don’t go outside unless I have to for work. I am so self conscious, I wear clothes that are too big for me, which makes me look bigger. I also cannot bend over, as my back hurts. 
I have since lost 5 kilos, and am dieting, but the weight of my breasts are a struggle for me and I feel my only option is surgery.
I have private health insurance and have made an appointment with my doctor.
My insurance company told me that I have to give them a SEF form and that I may not be covered.
I really don’t understand it. 
Any advice.
