i am a f to ff in cup size..doesnt matter what weight i am (although i am losing well in preparation for surgery)..i had a breast lift 14 years ago and within last 5 years have had osteomylitis badly in one shoulder resulting in on and off nerve pain now and 3 scars over the shoulder and piece of scapula gone…what size can i realistically go down to ?

Hello and thanks for your question.  It is almost impossible to give you a reliable answer without a face to face consultation.  Most Plastic Surgeons will attempt to match a patient’s final breast size to their build at the time of surgery. 
If you have had a previous breast lift, this presents additional challenges for your surgeon and increases the risk of secondary surgery, especially with respect to the sensation and blood supply to your nipple. These risks are increased with higher amounts of breast reduction (The more you remove the greater the risk).
Provided you are a relatively healthy weight (BMI under 32 in my practice) and have functional symptoms related to your breasts, a breast reduction procedure should help you and may be covered in part by Medicare and your private insurance.  
You should see a Plastic and reconstructive Surgeon who is a member of the Australian Society of Plastic Surgeons for a face to face consultation to discuss your options.
